If so, this unique opportunity is for you! We are in search of a dedicated Registered Nurse licensed in Wisconsin to join our team as an IRIS Self-Directed Personal Care (SDPC) RN. This remote position allows you to connect with community members enrolled in the IRIS program, a Medicaid long-term care option designed for older adults and individuals with disabilities. In this role, while primarily home-based, you will make regular visits to people's homes and communities. As an IRIS SDPC RN, your responsibilities will include providing oversight and guidance for IRIS participants, fostering meaningful relationships, and ensuring they benefit fully from the Self-Directed Personal Care option through assessments, oversight, training, and education. Your key responsibilities will include: • Conducting personal care assessments and overseeing My Cares Groups through the Wisconsin Personal Care Screening Tool. • Documenting assessments per Department of Health Services standards, completing required oversight visits, and follow-ups. • Developing individual plans of care in collaboration with participants, ensuring alignment with physician orders. • Submitting authorization requests for personal care services as necessary. • Strictly adhering to Department of Health Services policies, SDPC guidelines, and program regulations. • Providing training and educational materials to participants or their care providers as needed. • Communicating with IRIS Consultants, Long-Term Care Functional Screeners, and physicians to support care needs. • Completing additional duties as assigned, which may include overtime work. • Traveling up to 50% of the time for member home visits, navigating various home environments. Required Qualifications • A minimum of 2 years of nursing experience with at least 1 year serving the IRIS target groups (adults with physical/intellectual disabilities or older adults), or an equivalent combination of relevant education and experience. • Active and unrestricted Registered Nurse (RN) license in Wisconsin. • Associate's degree in nursing. • Valid driver's license, reliable transportation, and adequate auto insurance for job-related travel. • Proficient in database operations and data entry. • Strong teaching, mentoring, analytical, and problem-solving abilities. • Excellent organizational and time-management skills, with the ability to work independently. • Flexibility and adaptability to organizational needs. • Exceptional written and verbal communication skills. • Proficiency in Microsoft Office and related software. Preferred Qualifications • Experience with the Wisconsin Medical Assistance Personal Care program (MPAC). • Experience in home care or home health settings. Pay Range: $26.41 - $51.49 per hour.
