In the role of Lead Hospice Care Manager, RN, your responsibilities will include:
• Reporting directly to the Executive Director or Administrator.
• Leading and supporting a high-performing hospice program to ensure exceptional care delivery and compliance with regulations.
• Supervising, teaching, evaluating, and developing nursing and clinical associates to promote clinical excellence and professional growth.
• Managing the scheduling and assignment of admission and evaluation visits, ensuring timely clinical staff visits.
• Overseeing clinical and administrative workflows, including referral documentation, DME pickups, Medicare eligibility, insurance verifications, and patient benefit tracking.
• Coordinating and leading Interdisciplinary Group (IDG) meetings, including patient reviews and care plan updates.
• Monitoring adherence to company policies, procedures, and regulatory standards.
• Acting as a key liaison between clinical teams, leadership, physicians, and other providers for seamless, quality patient care.
What you will bring to the team:
• A current RN license in the state of employment.
• A graduate of an accredited nursing program.
• A minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care.
• Strong managerial skills, with proven experience in clinical operations and patient care coordination.
• A solid understanding of hospice principles, industry regulations, and best practices.
• Ability to lead with empathy and confidence while maintaining quality and operational excellence.
Preferred Qualifications:
• Bachelor of Science in Nursing (BSN).
• Previous experience in a clinical supervisory or program management role.
• CHPN certification (Certified Hospice and Palliative Nurse).
• Proficiency in EMR systems and healthcare technology platforms.
