Join our dedicated team at Notre Dame At Home - Hospice as a Clinical Team Lead. This full-time position (40 hours) plays a crucial role in providing compassionate and comprehensive care to patients nearing the end of life and their families. Our hospice services focus on delivering comfort, companionship, and support when curative treatments are no longer an option.
As the Clinical Team Lead, you will oversee an interdisciplinary team composed of Nursing, Social Work, Spiritual Care, Home Health Aides, Music Therapy, and Massage Therapy. Your leadership will ensure that all care aligns with established standards of practice, state and federal regulations, and our organizational protocols. You will promote clinical excellence, encourage timely and accurate documentation, and foster a collaborative team environment.
Key Responsibilities:
• Ensure that each hospice patient and family receives a comprehensive and interdisciplinary array of services.
• Lead the team to operate with a unified, patient-centered approach.
• Support staff in providing compassionate, efficient, and cost-effective patient care.
• Maintain professional standards and oversee the performance of the interdisciplinary team.
Requirements:
• Graduation from an accredited nursing program; a Bachelor of Science in Nursing (BSN) is preferred.
• Active and unrestricted Registered Nurse (RN) license in Massachusetts.
• Minimum of five (5) years of hospice nursing experience is required.
• Certified Hospice and Palliative Nurse (CHPN) credential is preferred.
• Prior experience in hospice leadership or management is strongly preferred.
• Demonstrated ability to work effectively within an interdisciplinary team.
• Proven leadership skills to foster collaboration and guide team performance.
• Strong commitment to the principles of hospice care and excellent clinical assessment and symptom management abilities.
• Exceptional interpersonal and communication skills to build rapport with staff, patients, families, and external partners.
• Valid driver's license and access to reliable transportation.
• Willingness to participate in continuing education to maintain licensure and stay current with best practices.
