Job Description
A company is looking for a Program Manager Advance Care Planning in Population Health.
Key Responsibilities
Coordinates daily operations related to healthcare ethics, palliative care, and advance care planning
Implements system-level advance care planning processes and monitors progress towards departmental goals
Provides education to care teams and collaborates with stakeholders to enhance advance care planning initiatives
Required Qualifications
Master's degree in a relevant specialty (social work, nursing, behavioral health, etc.)
Licensure appropriate to clinical / care specialty (CNS, LSW, LISW, LCSW, or RN required)
4 years of clinical practice experience in a care setting
ACP Instructor Certification preferred
Preferred education includes MSW or BSN in relevant specialty areas
