Job Description
A company is looking for a Care Coordinator - Remote.
Key Responsibilities
Coordinates care for clients with behavioral health conditions through assessments, care planning, and monitoring
Develops and implements care plans, ensuring appropriate services are rendered and advocating for members' needs
Collaborates with interdisciplinary teams to adjust care plans and improve health outcomes
Required Qualifications, Training, and Education
3-5 years of experience in Social Work, Nursing, or a related healthcare field
Experience in utilization management, quality assurance, or community health
Knowledge of cost-effective care coordination and referral processes
Ability to analyze data and make complex decisions
High School diploma or GED required; Associate or Bachelor's degree preferred
