We are seeking a dedicated and compassionate Licensed Nurse with specialized expertise in Memory Care and Assisted Living to join our growing team. Regional Role:
• Communities in Tucson, Phoenix and Surprise. You'd be expected to travel between the three areas.
Responsibilities
• Assisted Living licensure compliance, adherence to policies, and quality assurance.
• Deliver comprehensive nursing assessments focusing on memory care needs, including vital signs, physical examinations, and cognitive evaluations.
• Develop and implement individualized care plans for residents with Alzheimer’s disease, dementia, or other memory-related conditions, coordinating with interdisciplinary teams for optimal outcomes.
• Monitor residents’ behavioral health and respond promptly to acute changes or emergencies using triage skills and critical care experience.
• Manage medical documentation meticulously within EMR (Electronic Medical Records) systems such as Point Click Care.
• Provide education and support to residents’ families about disease progression, medication management, and safety strategies in memory care settings.
• Maintain infection control practices and aseptic techniques during all patient interactions while ensuring a safe environment for both residents and staff.
Experience
• Valid licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current state certification.
• Proven experience in long-term care facilities, assisted living communities, or nursing homes specializing in geriatrics or dementia care.
• Demonstrated expertise in Alzheimer’s care, dementia management, and behavioral health interventions.
• Ability to work effectively with diverse populations including individuals with developmental disabilities or those requiring hospice or post-acute care services.
