Riverside Health & Rehabilitation, a dedicated skilled nursing community, is excited to announce openings for Full-time Licensed Practical Nurses (LPNs) to join our dynamic clinical team!
SHIFT: Flexible 12 & 8 hour shifts available!
We offer Full-time, Part-time, or PRN positions across Days, Evenings, or Night shifts, with Rotating weekends.
WAGE: $7,000 Sign-On Bonus for Full Time LPNs!
Hourly rates range from $29/hr - $34/hr with an enticing $7,000 sign-on bonus for full-time roles.
The primary responsibility of a Licensed Practical Nurse is to deliver high-quality, holistic nursing care while adhering to established nursing practice standards and regulations. In this role, you will lead and guide both licensed and non-professional staff in providing direct resident care and crucial support functions.

Essential Job Functions:
Nursing Care Responsibilities:
• Conduct daily nursing functions in line with current regulations and guidelines governing long-term care facilities.
• Ensure compliance with written nursing policies and safety protocols by all nursing staff under your supervision.
• Manage the admission, transfer, and discharge processes of residents, maintaining accurate documentation.
• Participate in educating new residents and their families about facility services.
• Conduct rounds with health care providers and interdisciplinary staff as required.
• Utilize nursing staff in accordance with residents' care and service plans.
Communication and Documentation Responsibilities:
• Complete daily clinical rounds to monitor resident care and promptly report significant findings.
• Address and resolve issues raised by residents and family members, escalating major concerns to management.
• Engage in facility audits and surveys as directed by management.
• May oversee specific clinical programs including dementia care, wound care, and infection control.
• Assist in developing care plans and perform assessments as necessary.
• Maintain accurate and complete medical records and documentation.
Medication Administration Responsibilities:
• Prepare and administer medications as per healthcare provider orders.
• Collaborate with pharmacy to ensure medication availability.
• Manage inventory of medications and necessary supplies to meet residents' needs.
• Ensure proper documentation for medication administration is maintained.
Personnel Responsibilities:
• Supervise nursing staff to ensure adherence to professional standards.
• Work alongside the Director of Nursing to address performance issues.
• Facilitate the communication of staff issues to management and collaborate on resolutions.
Safety and Sanitation Responsibilities:
• Ensure availability of protective equipment for all staff.
• Maintain cleanliness and sanitation in nursing service areas.
• Recommend needs for medical supplies and equipment to the Director of Nursing.
• Perform other job-related duties as needed.
Qualifications:
• Meet all applicable state and federal requirements for this position.
• Hold a current and active LPN/LVN license in good standing.
• A minimum of one year experience in nursing within a home care, assisted living, or long-term care setting is preferred.
Supervisory Responsibility:
This role involves ensuring compliance with care standards and guiding CNAs/Caregivers in delivering resident care, although they do not report directly to the LPN.
