LPN JOB DESCRIPTION
JOB SUMMARY
The Licensed Practical / Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
DUTIES & RESPONSIBILITIES
Provides direct patient care as defined in State's Nurse Practice Act.
Implements plan of care initiated by the registered nurse.
Provides accurate and timely documentation consistent with the plan of care.
Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s).
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
Uses equipment and supplies effectively and efficiently.
Participates in personal and professional growth and development.
Performs other duties as assigned by the registered nurse.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
Must have completed a requisite practical (vocational) nursing program or be licensed in the state where the HHA is located.
Has three years nursing experience preferred. Community/home health or medical/surgical experience is preferred.
Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
Complies with accepted professional standards and practice.
Demonstrates good verbal and written communication, and organization skills.
Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
Possesses and maintains current CPR certification.
WORKING ENVIRONMENT (Some of the following items may apply)
Blood & Bodily Fluids, Latex, Odors, Chemicals, Disease, Hazardous Drugs
Other: TB (to require mask)Mechanical/Electrical
PHYSICAL REQUIREMENTS -- based on a normal day's work.
Maximum weight expected to lift will be 100 lbs
Frequent weight expectation will be 50 lbs
Occasional weight expectation will be 20 lbs
Activities:
Sitting / Standing / Left/Right foot movement |
Walking = Limited 20%-50% of dayTwisting |
Bending / Squatting / Push/Pull Objects = Occasional Up to 20% of day
Reaching Above and/or Below shoulder level = Occasional Up to 20% of day
Handing (holding, grasping, working with hands) = Occasional Up to 20% of day
Repetitive movement of the hand (includes typing and ten-key use) = Occasional Up to 20% of day

Senses needed to perform essential functions of the position: Hearing, talking, depth perception, color, vision, touch and smell.
Maintain a stable posture and gait with hands free to perform anticipated or routine and emergent patient care.
Endurance (cardiovascular fitness)Physically and mentally able to work extended hours when necessary
