Department Details: • Shift: 6pm - 6am • Inclusive of rotating weekends and holidays • Shift differentials available for night and weekend shifts Job Summary: The Long Term Care Licensed Practical Nurse (LPN) at St. Luke's plays a vital role in providing professional nursing care for residents of all ages in a long-term care setting. Under the supervision of a registered nurse, advanced practice provider, or physician, you will: • Demonstrate knowledge of nursing principles relevant to resident care. • Utilize expert nursing capabilities to meet physical and psychosocial needs as per care plan. • Administer medications and treatments as ordered, contributing to health, comfort, and safety. • Document all resident interactions and care provided effectively. • Communicate clearly and establish strong relationships with residents and the healthcare team. • Counsel residents and family members on treatment and prevention plans. Qualifications: • Graduate from an accredited practical nursing program; an associate degree is preferred. • Must hold an unencumbered Licensed Practical Nurse (LPN) license. • Experience of at least 400 hours as an LPN or completion of a supervised clinical program. • Maintain competencies and certifications as required by the department.
