As a Home Care LPN, you will: • Deliver professional nursing services under the guidance of an RN, adhering to the state's Nurse Practice Act, licensure requirements, healthcare standards, and company policies. • Provide skilled nursing interventions focusing on illness treatment, rehabilitation needs, and preventive care, using a holistic approach. • Apply your nursing knowledge and skills according to established clinical practices to effectively resolve issues and achieve patient-specific goals. • Consult with the RN Supervisor to adjust the Plan of Treatment as necessary, receiving verbal orders from physicians and communicating these to the RN Supervisor. • Utilize a systematic and patient-centered approach to implement the nursing plan of care. • Maintain timely communication with patients, physicians, clinical managers, and other healthcare team members regarding patient/family needs and status changes. • Uphold the highest standards of professional integrity concerning confidential information, sharing it only when appropriate and beneficial to the patient. • Communicate effectively to ensure the availability and safety of supplies and equipment necessary for the care plan. • Document all care provided as required by company standards, ensuring notes are recorded on the same day services are delivered. Required Skills and Experience: • Graduation from an accredited Licensed Practical Nursing Program or equivalent vocational school. • Current nursing license in the state of practice. • Valid driver's license, auto insurance, and reliable transportation. • Current CPR certification. • At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice. Compensation: Pay is structured at a rate of $28.00 - $40.00 per visit/unit, with a base salary range of $44,600 - $61,400 annually.
