Licensed Practical Nurse Home Health - Greenwood, South Carolina in Greenwood, SC

An opportunity is available for a Licensed Practical Nurse (LPN) with home health experience to provide compassionate, patient-centered nursing care in a home setting. This role demands clinical expertise, strong communication, and the ability to manage a caseload independently while delivering high-quality care that promotes patient independence and recovery. Schedule & Type: • Full-time contract position • Monday to Friday, 8:00 AM 5:00 PM with occasional flexibility for patient needs • Field-based, conducting 35 patient visits daily within an assigned territory Key Responsibilities: • Perform skilled nursing visits to assess, develop, and implement individualized care plans • Administer medications, wound care, and clinical interventions such as IV therapy • Monitor patient conditions and communicate changes promptly to care management • Educate patients and caregivers on disease management and self-care best practices • Maintain accurate documentation using Home Care Home Base (HCHB) • Collaborate with interdisciplinary teams to ensure care coordination and compliance • Uphold infection control and patient safety standards • Participate in case conferences and ongoing training Qualifications: • Active, unrestricted LPN license in the state of practice • Minimum one year of home health, hospice, or community-based care experience • Proficiency with Home Care Home Base (HCHB) is required • Valid drivers license, reliable insured vehicle • Current Basic Life Support (BLS) certification • Strong clinical judgment, time management, and communication skills • Ability to work independently in a field setting Preferred: • Experience with Medicare-certified home health agencies • Deep understanding of home health compliance and documentation • Excellent interpersonal skills focused on patient-centered care Benefits include medical, dental, and vision insurance options, 401(k), weekly pay, and a customer support line available 24/7.
