Licensed Practical Nurse - Home Health Make a difference daily in your role as a nurse within our home healthcare services. You'll offer personalized care, aiding patients in their journey to regain independence comfortably at home. Collaborate with a devoted team of medical professionals to create and manage care plans that support recovery and help patients resume their daily activities. Home Health LPN Responsibilities: • Deliver nursing services under RN supervision following state regulations and internal guidelines. • Offer skilled nursing care to address patient illnesses, rehabilitation needs, and preventive care, using a holistic approach. • Use professional judgment to assist in problem-solving and achieving patient goals. • Consult with the RN Supervisor about necessary adjustments to treatment plans, and communicate physician orders as needed. • Implement nursing plans with individualized, goal-driven strategies. • Communicate effectively with patients, medical staff, and team members to address care needs and changes. • Participate in care coordination and discharge planning. • Adhere to confidentiality standards and share patient information responsibly. • Ensure availability and proper use of necessary supplies and equipment. • Complete required documentation accurately and on time. Apply Your Expertise to Make a Difference Required Experience and Skills: • Completion of an accredited Licensed Practical Nursing Program. • Current nursing license. • Valid driver's license, auto insurance, and reliable transport. • CPR certification. • Two years of experience as an LPN/LVN in a clinical setting, ideally in home health or hospice care. Potential Salary: • Pay per Visit/Unit: $28.00 - $40.00 • Annual Base Pay: $44,600 - $61,400 Weekly Hours: • Full-time, 40 hours Pay Expectations: • The estimated base pay range for full time positions is between $53,000 and $73,000 annually.
