The primary purpose of this job position is to provide oversight and management to the provision of service delivery to participants. Provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders. Services provided in the home, the community and on site.
Position Purpose:
This position provides skilled nursing care and manages service delivery to participants, ensuring adherence to professional standards and primary care orders.
Position Details:
Position will include work in both our clinic as well as completing home care visits. Duties will include home care assessments and helping in the day center as needed.
What you will do:
• Provides skilled nursing care and prescribed treatments per professional standards and primary care orders.
• Demonstrates and instructs participants and families in treatments and self-care.
• Participates in program activities and develops comprehensive care plans to promote health, independence, and self-care.
• Communicates with and counsels participants and families, making referrals as needed.
• Assists with medication management and documentation.
Minimum Qualifications:
• Must be a graduate of an accredited Nursing Degree program and possess a current, unencumbered, active license to practice as a Licensed Practical Nurse.
• Active CPR Certification required.
• Two (2) years of experience with geriatric populations.
• Must have a valid driver's license, insurance and use of personal vehicle for each shift due to home care assignments.
• Ability to perform standard clinical procedures accurately and document services thoroughly.
• Superior written and verbal communication skills.
• Proficiency with MS Office. Ability to learn/use other software as required.
• Must possess excellent interpersonal, problem solving, conflict management, customer service and organizational skills.
• Must be flexible to adapt to ongoing change, be able to prioritize workload, and exercise independent judgment.
