We are expanding our team and are seeking compassionate, organized, and team-oriented Licensed Vocational Nurse (LVNs) for multiple territories. Compensation is based on experience and territory assignment: Alameda County & West Contra Costa County Pay Rate: $45–$55 per hour DOE Coverage Area: Oakland/San Leandro/Berkeley/Richmond/San Pablo East Contra Costa County Pay Rate: $45–$55 per hour DOE Coverage Area: Antioch/Pittsburg/Brentwood TriValley Area Pay Rate: $45–$55 per hour DOE Coverage Area: Dublin/Livermore/Pleasanton/San Ramon/Danville Why Health Link Strong, experienced clinical support team available whenever you need assistance Flexible scheduling Competitive compensation packages Medical, Dental, Vision, 401(k) with matching, Life Insurance, and more Paid Time Off (up to 10 days), accruing and available for your use Vehicle reimbursement options Supportive and collaborative back-office staff Responsibilities Implement the plan of care as initiated by the Registered Nurse Provide accurate, timely, and complete documentation in accordance with the plan of care Assess patient needs and deliver patient and family/caregiver education related to the diagnosis, treatment, and plan of care Participate in coordinating home health services, including identifying needs for additional disciplines (HHA, OT, PT, ST, MSW) and communicating them to the Registered Nurse and/or Clinical Supervisor Use equipment and supplies effectively and efficiently Complete online charting in Kinnser promptly and accurately Adhere to all company policies and procedures Perform additional duties as assigned Why You’ll Be Successful You prioritize patient-centered care You enjoy teamwork and community partnership You value appreciation and support from your colleagues Requirements Current California LVN License (Required) Home Health Experience: 1 year (Preferred) Current CA Driver’s License Excellent organizational and communication skills Ability to work independently in the field If you are passionate about providing exceptional care and would like to join a dedicated team that values your expertise, apply today and let us know which territory you prefer!
