Job Description
Overview
Licensed Vocational Nurses (LVNs) help fulfill the mission of People's Community Clinic by promoting and facilitating excellent health care for patients.
This position works as a member of a care team to coordinate patient care and support patients and families in self-management, self-efficacy, and behavior change. Care team members are expected to assess and manage the health needs of the patient population and to communicate effectively with all patients, especially vulnerable populations.
North Clinic Location: 1101 Camino La Costa, Austin, TX 78752
Compensation: $24.50 - $30.50 hourly based on experience and qualifications
Responsibilities
ESSENTIAL FUNCTIONS:
Serve as a resource and subject-matter expert for Medical Assistants, providing guidance and support as needed.
Actively participate in care team huddles, staff meetings, and collaborative problem-solving related to clinical operations.
Assist in clinic operations by working on the floor as necessary to ensure efficient patient flow, including but not limited to: rooming patients, gathering focused patient information, vital signs, point of care testing, immunizations, medication administration, procedures, and patient call backs.
Assist with any clinic emergencies within the scope of practice.
Manage and complete medication prior authorizations.
Manage and complete prescription refill requests according to delegated or standing orders.
Make patient follow-up calls as instructed by providers or per delegated orders, including communicating laboratory and diagnostic test results to patients as directed.
Complete provider-directed tasks such as managing paperwork requiring provider review or signature, ensuring timely processing and communication.
Educate patients on disease processes, medication management, and health promotion strategies.
Facilitate communication among patients, providers, nurses, and other healthcare professionals.
Identify gaps in patient care and collaborate with the care team to address and close them.
Clinical Compliance and Quality Improvement:
Participate in the training of Medical Assistants.
Identify and support opportunities for workflow and quality improvement initiatives to enhance patient care.
Strictly adhere to HIPAA guidelines, organizational policies and procedures, including standing delegation orders.
Qualifications
Education:
High school diploma or GED required
Graduation from an accredited vocational nursing program
Experience:
Two years of healthcare experience preferred
One year in a primary care clinic setting strongly preferred
Knowledge, Skills, and Abilities:
Bilingual English/Spanish preferred.
Proficient in basic LVN nursing skills and laboratory procedures.
Strong attention to detail and superior customer service skills.
Excellent verbal, written, and telephone communication abilities.
Effective multitasking, time management, and task prioritization skills.
Competent in the use of computers and electronic medical records systems.
Ability to establish and maintain positive working relationships with individuals from diverse backgrounds.
Capable of working independently while successfully accomplishing assigned tasks.
License/Certification: Current, unrestricted LVN license from the Texas Board of Nursing required.
