Job Description:
• Seeking 2 full-time Registered Nurses (RNs) for a permanent Home Health position in Milwaukee, WI.
• The Home Health RN provides skilled nursing care to patients in their homes, focusing on assessment, treatment, education, and care coordination.
• This role requires independent clinical judgment while collaborating with physicians, therapists, and the interdisciplinary care team to promote patient safety, recovery, and independence.
Responsibilities:
• Perform comprehensive in-home patient assessments (physical, emotional, and environmental)
• Develop, implement, and update individualized plans of care
• Provide skilled nursing services in accordance with physician orders
• Monitor patient condition and identify changes requiring timely intervention
• Educate patients and families on disease processes, medications, and care plans
• Coordinate care with physicians, therapists, and interdisciplinary team members
• Work independently in the field while ensuring patient safety and quality outcomes
Skills:
• Strong clinical judgment and decision-making skills
• Ability to work independently without immediate on-site support
• Effective prioritization and time management across multiple patient visits
• Ability to respond rapidly to changes in patient condition
Requirements:
• 1 year of RN Home Health experience
• Bachelor's BSN degree
• Compact or WI State RN License
