This hybrid role includes both field and virtual assessments and is available across New York State, including Downstate (NYC, Long Island, Westchester) and Upstate regions. Position Overview: As a NYIA RN Assessor, you will conduct comprehensive in-person and virtual assessments to determine eligibility for community-based long-term care services. You will utilize the UAS-NY tool to assess medical, functional, and psychosocial needs, ensuring accurate, compliant documentation. Minimum Requirements: • Active RN license in New York State • 2+ years RN experience in home care, assessments, or community health • Experience with the UAS-NY Community Assessment Tool • Valid New York State–issued ID • Strong clinical assessment and documentation skills • Ability to travel for field assessments (statewide) This is an ongoing NYIA RN Assessor position with no end date, available statewide across Downstate and Upstate NY.
