
Londonderry Hospice Triage Registered Nurse (Weekends/Day Time) in Derry, NH

Position Details:
• Main Office location: Londonderry, NH
• Service Area: Londonderry and Surrounding areas
• Job Type: Part-time
• Schedule: Saturday and Sunday 8:30 am – 8:00 pm
Responsibilities:
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
Qualifications:
• 2+ years' nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
Benefits & Compensation:
• Pay Structure: Base Compensation ($600 per weekend) + Paid per visit ($50) and per admission ($100). Visits over 60 minutes are paid at $75. Training and meetings are paid hourly at $35/hr.
• Mileage Reimbursement ($0.36/mile)
• 401(k) Match.
