Job Description
A rewarding opportunity is available for a Registered Nurse specializing in Rehabilitation and Long-Term Care to support patients in nursing home and assisted living settings. This full-time contract role offers a Monday through Friday schedule, 8 AM to 5 PM, with no weekends or holidays, providing a balanced work-life experience.
In this position, you will deliver comprehensive care for insurance members residing in skilled nursing and assisted living facilities. The goal is to manage patient care proactively to avoid unnecessary hospitalizations and support successful transitions back to the home environment. Regular health assessments are performed at least monthly, or more frequently as needed, to enhance healthcare outcomes and reduce emergency room visits.
Responsibilities include:
• Collaborating closely with Nurse Practitioners and interdisciplinary care teams to coordinate member care.
• Performing routine monthly visits and resident rounds to assess progress toward health goals.
• Attending meetings and communicating updates to the care team.
• Facilitating care coordination activities within skilled nursing facilities to address resident needs.
• Assisting with clinical and quality initiatives, including identifying and addressing gaps in care.
• Engaging effectively with residents, families, facility staff, and the care team.
• Supporting Advanced Care Planning and acute visit coverage at designated locations.
Qualifications:
• ASN or BSN degree.
• Current New Hampshire RN license.
• BLS certification.
• Minimum of one year experience in Long-Term Care, Care Coordination, or Care Management preferred.
