Job Description
Key Responsibilities:
• Conduct thorough patient assessments to determine suitability for hospice care and ongoing learning needs.
• Develop, implement, and evaluate evidence-based plans of care, revising as needed to meet dynamic patient needs.
• Maintain productivity standards and demonstrate knowledge of insurance reimbursement practices.
• Complete admission, discharge, and revisit documentation promptly using electronic systems.
• Apply age-appropriate teaching methods for patients and families, ensuring education plans are effective and documented.
• Coordinate interdisciplinary communication and participate in bi-weekly team meetings.
• Ensure patient and staff safety through adherence to infection control and safe medication management protocols.
• Participate in performance improvement initiatives, mentoring new staff, and cross-training to expand skills.
• Support departmental growth through orientation and mentorship activities.
Desired Experience and Qualifications:
• Valid Registered Nurse (RN) license with experience in rehab and long-term care settings.
• Strong understanding of hospice care regulations and insurance guidelines.
• Proficiency in patient assessment and care planning.
• Excellent communication and interdisciplinary collaboration skills.
• Familiarity with electronic health record systems and documentation.
• Commitment to maintaining a safe care environment and adherence to infection control standards.
