Join our multidisciplinary team as a Care Manager for Long-Term Services and Supports (LTSS) and make a difference in the lives of members with high-need potential. In this role, you'll collaborate to coordinate integrated member care and work towards achieving quality, cost-effective health outcomes. Essential Job Duties • Conduct comprehensive member assessments within regulated timelines, including in-person home visits. • Facilitate comprehensive waiver enrollment and disenrollment processes. • Develop and implement care plans in collaboration with members, caregivers, and healthcare professionals. • Monitor care plans for effectiveness and make necessary adjustments to meet member goals. • Promote integration of services including behavioral health care and community resources. • Assess medical necessity and authorize appropriate waiver services. • Evaluate covered benefits and advise on funding sources. • Facilitate interdisciplinary care team meetings for service approvals. • Utilize motivational interviewing techniques to support and educate members. • Assist members in overcoming barriers to care related to psycho/social, financial, and medical issues. • Identify critical incidents and develop intervention plans for member safety. • Consult and provide resources to peers as necessary. • May manage complex member cases, including medication reconciliation as needed. Travel may be required for local visits, estimated at 25-40% based on state/contractual requirements. Required Qualifications • Minimum of 2 years of healthcare experience, including 1 year in care management or a medical/behavioral health setting. • At least 1 year of experience working with individuals facing disabilities, chronic conditions, and substance abuse disorders. • Current, active, and unrestricted Registered Nurse (RN) license. • Valid driver's license, reliable transportation, and adequate auto insurance. • Detail-oriented with proactive work ethic. • Strong knowledge of community resources and ability to adapt to diverse populations. • Independent worker with excellent communication skills and ability to remain calm under pressure. • Strong problem-solving and critical-thinking capabilities. • Proficient in Microsoft Office and applicable software programs. Preferred Qualifications • Certification as a Case Manager (CCM). • Experience with populations utilizing waiver services.
