Schedule: Full Time | 80 hours per pay period | 10am-6:30pm (flexible on hours)
• $2,000 sign-on bonus!!*
Training & orientation hours will be approximately 8am-4:30pm until training is completed.

As a Licensed Practical Nurse (LPN), you’ll be at the heart of our community, providing compassionate and skilled nursing care while fostering meaningful connections with residents. Whether administering medications, monitoring patient health, or mentoring Nursing Assistants, you’ll play a key role in enhancing the dignity, safety, and well-being of our residents.
As a LPN, you will:
• Monitor Resident Health and Document Changes – Obtain necessary information to initiate and update the Plan of Care for each resident. Observe and document changes in resident’s physical, mental, or emotional status. Communicate resident changes with the interdisciplinary team and document all observations.
• Provide Direct Client Care – Administer medications as prescribed or other physician-ordered treatments. Ensure accurate transcription of new physician orders and maintain clean, organized med carts and nursing stations.
• Communicate with Residents and Families – Educate residents about their medications, treatments, and side effects while respecting their right to make personal choices. Communicate changes in resident health to families, offering support and comfort during difficult times.
• Lead and Support – Monitor and provide work direction to Nursing Assistants, offering guidance, instruction, and mentorship.
• Other Duties as Assigned – The duties and responsibilities above are representative of the nature and level of work assigned and are not necessarily all inclusive.
This job also requires:
• Current Licensed Practical Nurse (LPN) license in the state of Minnesota.
• CPR certification required within 6 months of hire.
• Experience working with seniors in long-term care, home care, or assisted living settings is preferred.
• Ability to work effectively with a diverse population, including residents with varying values, backgrounds, and capabilities.
• Physical capability to perform job duties.
• Proficiency in reading, writing, and speaking English to communicate effectively with residents, families, and team members.
• Ability to pass state mandated background check.
