Job Description
What You Should Know About the LPN/RN Care Team Navigator Role:
• This is a FULL- time role 6pm-6:30am including every other weekend. 2 week rotation: Tuesday, Wednesday, Saturday, Sunday, Thursday, Friday.
• Holiday rotation required.
The LPN/RN Care Team Navigator Essential Duties are:
The Care Team Navigator will ensure that Ohio’s Hospice patients and families receive excellent care and services. The Care Transition Navigator will coordinate care and provide support to patients, families, and Ohio’s Hospice teammates.
Qualifications:
• RN’s - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred
• Active Registered Nurse license in the State of Ohio without any board actions.
• Ability to provide direct patient care
• Two years acute care nursing experience preferred
• Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’s Hospice employees who wish to attain certification.
• Must be able to pass a criminal background check
• Must be able to pass a 10 panel drug screen
