The LPN provides quality nursing care and services to residents in a respectful and dignified manner, ensuring the delivery of care and services. The LPN coordinates physical, social, and emotional aspects of assisted living care and services in accordance with the physician's orders and the resident's choices.
• Execution of the medication management plan including periodic review of the documentation, observation of the effectiveness of the plan, communication to the physician when changes are needed or outcomes are not appropriate
• Continuous assessment of resident needs with updates to appropriate individuals as it relates to care, services and changes in condition
• Implement measures to ensure resident wellness and safety and the appropriate documentation of those measures
• Work in a team fashion to meet resident needs
• Respond to emergencies timely and in a calm and competent manner
• Report incidents and changes in resident condition to Healthcare Director and with the appropriate documentation
• Be an active part of all activities including community events
• Be present and assist with the delivery of meals in the dining room, monitoring resident acceptance of diets and assuring adequate nutrition
• Maintain high standards with regard to record keeping, resident and staff documentation
• Establish and maintain a positive working relationship with all departments
• Identify risk factors for residents and initiate interventions including the appropriate documentation of that intervention
• Provide a thorough report of resident status to on-coming nursing team, at each change of shift, and in the Daily Log
• Provide motivation and direction to CNA/RMA's
• Must be certified or willing to take and pass the certification classes required by the state in which employed
• Attend required trainings, CEU's and staff meetings
• Adhere to and implement all policies and procedures
• Practice proper body mechanics and safe resident transferring techniques
• Adhere to attendance policies; report to work on time and prepared to begin duties
• Complete all other assigned duties
Requirements:
• Current LPN license to practice nursing in the state in which the community is located, holding a license in good standing with the Board of Nursing
• Experience working with the senior population, preferably in assisted living or LTC
• Must be a team player who works well with others and has experience supervising staff
• Must possess critical thinking skills, ability to effectively communicate verbally and in writing
• Demonstrated ability to manage time efficiently with strong organizational skills
• Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs
• Must be willing to work an 8-16 hour flexible schedule to include weekends and holidays
• Must attend all required trainings, CEU's and staff meetings
• Must be certified or willing to take and pass the certification class required by the state in which employed
