Position Overview Outfield Healthcare Partners is seeking a skilled MDS Coordinator (RN or LPN) to relocate to one of our New Mexico skilled nursing facilities. This role is responsible for accurate and timely completion of the RAI/MDS process, care planning coordination, and regulatory compliance from admission through discharge. You’ll work closely with the Director of Nursing and interdisciplinary team to support quality outcomes and reimbursement accuracy.
