Your Responsibilities:
• Lead the collaborative, interdisciplinary assessment and care planning process within our skilled nursing facility.
• Manage MDS assessments, rectify rejected assessments, and comply with Minimum Data Set (MDS) and resident assessment instrument (RAI) standards.
• Ensure all residents are evaluated according to required timeframes or status changes, in line with federal, state, and Medicare regulations.
What We Seek:
• Active RN License in Minnesota.
• Minimum of one year of nursing experience (required).
• Exceptional critical thinking skills, independent judgment, and the capacity to multitask and prioritize in a fast-paced setting.
• Proficient in reading, writing, and speaking English.
