We are seeking an experienced RN to serve as our Full-Time MDS Coordinator, joining the Nursing Administration Team to ensure compliance, optimize reimbursement, and promote person-centered care. • Participation in the on-call rotation as part of the facility leadership team is required. • Must have MDS experience We’re proud to be part of Catholic Charities New Hampshire, delivering care with dignity, purpose, and heart. Full Time Benefits; • Outstanding and affordable healthcare plan options with a Health Reimbursement Arrangement (HRA) to reimburse most of your out of pocket expenses. • Comprehensive medical, dental, and vision coverage • Earn additional pay of $2400 per year, if you are enrolled in another employer-sponsored health insurance plan • Critical illness, accident, long-term disability, legal plan and pet insurance available • Company-paid short-term disability and life insurance • Flexible Spending Account· 401K Plan, with 3% automatic employer contribution (after 1 year of employment) Key Responsibilities: • Lead the RAI/MDS process including person-centered care planning, PDPM reimbursement, Medicare certification, scheduling, month-end triple check, and appeals • Maintain PDPM/Medicare and Medicaid assessment schedules and distribute to therapies, dietary, social service, activities, and nursing teams • Provide ongoing education and training on MDS and care planning • Oversee the RAI process, including MDS assessments, triggers, CAAS, and care planning • Maintain Medicare certification/recertification binder to ensure compliance with federal Part A & B services • Stay current on long-term care regulations, best practices, and reimbursement requirements • Support the facility in audit and educational tools, including template development • Utilize Microsoft Office (PowerPoint, Word, Excel, Outlook) for reporting, tracking, and training Requirements • Registered Nurse (RN) with an active New Hampshire license • RAC-CT certification preferred • Minimum of 2 years’ experience in long-term care, including experience with the RAI process and person-centered care planning • Strong clinical judgment and excellent communication skills • In-depth knowledge of the RAI process and federal and state reimbursement requirements • Highly organized with the ability to manage multiple priorities throughout the day • Proficient in Microsoft Word, Excel, PowerPoint, and Outlook • Experience creating templates and tools for audits, tracking, and staff education • Participation in the on-call rotation as part of the facility leadership team is required All qualified applicants will receive consideration for employment without regard to race, color, religion, sex or national origin, marital or familial status, age or mental or physical handicap.
