MDS COORDINATOR JOB DESCRIPTION
Schedule:MondayFriday, 8:00am4:30pm
Status:Full-Time
Reports To:Director of Nursing / Administrator
On-Call:Participates in the weekend on-call rotation
Location:Coventry Respiratory & Rehab (or facility name)
Position Summary
The MDS Coordinator is responsible for overseeing and managing the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of the MDS, care plans, and related documentation. This role supports compliance with state and federal regulations, drives appropriate reimbursement through PDPM accuracy, and collaborates closely with the Interdisciplinary Team (IDT) to deliver high-quality resident-centered care.
The MDS Coordinator may be required to work the floor in the event of a staffing shortage, supporting resident care needs as assigned.
Key Responsibilities
MDS & Care Planning
Accurately complete and submit MDS assessments within required timeframes.
Develop, update, and coordinate individualized care plans in collaboration with the IDT.
Ensure compliance with PDPM guidelines, supporting accurate coding and reimbursement.
Monitor assessment schedules and maintain required documentation according to regulatory standards.
Clinical Collaboration & Communication
Work closely with nursing, therapy, social work, and other departments to ensure accurate, consistent resident assessment data.
Participate in care conferences, morning meetings, and QAPI as needed.
Communicate changes in resident status and collaborate to update care plans accordingly.
Regulatory Compliance
Ensure all assessments and documentation meet CMS, state, and federal guidelines.
Maintain up-to-date understanding of RAI/PPS regulations and PDPM updates.
Support survey readiness and participate in survey preparation activities.
Additional Responsibilities
Provide clinical support on the nursing floor during staff shortages or emergencies.
Participate in the weekend on-call rotation.
Assist with training and education of staff related to MDS processes and documentation best practices.
Required Qualifications
Current RN license in good standing(required).
Experience with MDS, care planning, and PDPM preferred.
Strong assessment and clinical judgment skills.
Excellent documentation, organizational, and time-management abilities.
Strong written and verbal communication skills.
Ability to collaborate effectively with the interdisciplinary team.
Detail-oriented, with the ability to maintain regulatory compliance and accuracy.
Preferred Skills & Attributes
Familiarity with EMR systems and MDS software.
Ability to multitask in a fast-paced environment.
