You ensure that every resident has a roadmap to wellness (Care Plans) and that the facility is accurately recognized for the high-level care we provide through the Minimum Data Set (MDS). Your Key Responsibilities • MDS Management: Coordinate, record, and monitor all MDS assessments in strict accordance with CMS guidelines and PPS rules. • Care Planning: Lead the development of interdisciplinary, written plans of care with measurable objectives for every resident. • Financial Stewardship: Understand and apply concepts of RUG IV and state case mix to ensure accurate reimbursement and avoid payment penalties. • Family & Team Liaison: Coordinate patient/family care conferences and act as the primary communicator between medical staff, families, and payor sources. • Quality Oversight: Monitor quality metrics and reporting programs to identify trends and implement improvement actions. • Staff Education: Empower your team by providing education on supportive documentation and late-loss ADLs. Qualifications • Education: Graduate of an accredited RN or LPN program. • Licensure: Active Arizona State RN or LPN license. • Certification: CPR certification (required upon hire). • Experience: 6 months in a long-term care environment preferred. • Skills: Expert knowledge of CMS guidelines, excellent multitasker, and a "person-centered" approach to care documentation.
