
Med-Surg Nurse (RN) - Brooklyn, NY in Guttenberg, NJ
Job Description
Nursing degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.
Providing skilled nursing care in patients homes, this role requires coordination and direction of nursing services in accordance with physician orders and agency protocols. You will be responsible for supervising and evaluating Home Health Aides (HHAs) and Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs), ensuring comprehensive patient care within regulatory guidelines. This contract position requires reliable transportation and occasional office meetings, with most work taking place during home visits.
Responsibilities include:
• Conducting patient assessments and reevaluations to tailor and revise plans of care
• Supervising and evaluating HHAs and LPNs/LVNs regularly, meeting state-specific requirements
• Communicating significant changes in patients medical conditions to physicians promptly
• Maintaining detailed, timely documentation in patient medical records
• Adhering to all federal and state home healthcare regulations and professional nursing standards
• Participating in ongoing in-service education to support agency staff development
• Coordinating multidisciplinary services to support holistic patient care
Qualifications:
• Current, unencumbered Registered Nurse (RN) license valid in the assigned state; Nurse Licensure Compact eligibility preferred
• Minimum 2-3 years of nursing experience, preferably in home health or related fields
• Graduate of an accredited nursing program; BSN preferred for Illinois assignments
• Current CPR certification required
• Strong assessment skills with the ability to address age-specific patient needs
• Proficiency with basic computer operations including email, internet navigation, and online training platforms
• Must possess a valid drivers license, reliable transportation, and meet agency liability insurance requirements
