A healthcare opportunity is available for experienced Registered Nurses with Medical-Surgical or ER/ICU backgrounds to provide patient-centered care in a dynamic, hybrid role that combines command center nursing and in-home patient support. The position focuses on delivering hospital-level care in the comfort of patients’ homes as part of an innovative Hospital Care at Home program. This role requires nurses to support the care team both remotely and through direct patient interaction in Newark, DE. Key Responsibilities: • Deliver comprehensive nursing care to patients in their homes, adhering to hospital-level protocols. • Provide clinical support and coordination from the command center to ensure seamless patient care. • Manage patient assessments, medication administration, and interventions following medical guidelines. • Collaborate with interdisciplinary teams to optimize patient health outcomes. • Participate in shift rotations including weekends and varying hours between 7:00 AM and 11:00 PM. • Maintain accurate clinical documentation and communicate effectively within the care team. Qualifications and Desired Experience: • Current and valid Registered Nurse (RN) license. • Experience in Medical-Surgical nursing or Emergency Room/Intensive Care Unit settings. • Strong clinical assessment and critical thinking skills. • Ability to work independently in patients’ homes as well as collaboratively in a command center. • Flexibility to work a 12-hour shift schedule, including weekends and rotations. • Comfort with travel between patient homes throughout the shift.
