Provide specialized nursing care in hospice and home health settings with a focus on comprehensive, compassionate end-of-life support. This opportunity involves caring for terminally ill adult patients primarily within an inpatient respite house and across home and long-term care environments. The role requires skilled symptom management, medication administration, and patient/family education, ensuring quality comfort care. Key Responsibilities: • Deliver hospice nursing services to patients in inpatient and home/SNF/ALF settings. • Manage complex symptom control including pain management, continuous infusions, and palliative sedation. • Perform specialized procedures such as central line care (PICC, PORT/IVAD), tracheostomy management, wound and ostomy care. • Conduct thorough patient assessments and develop individualized care plans. • Communicate effectively with patients, families, and interdisciplinary team members about goals of care. • Utilize electronic documentation systems for accurate record-keeping. • Participate in rotating schedules covering approximately 12-hour shifts both inpatient and home hospice settings, including weekend shifts. • Collaborate with advanced practice clinicians and ancillary staff to optimize patient outcomes. Desired Qualifications: • Registered Nurse with 3-5 years clinical experience, including hospice or related specialty. • Valid BLS certification required. • Comfort with technology such as CADD pumps, suction devices, respiratory support equipment. • Strong critical thinking and symptom assessment skills. • Experience with end-of-life care planning and patient-family education. • Ability to work independently and within a team across varied care environments.
