Medical-Surgical Nurse (RN) - Garden City, New York in Albertson, NY

Provide skilled nursing care in patients homes following physician orders and agency protocols. This contract role is based in Garden City, NY, offering an opportunity to apply your Registered Nurse expertise in a home health setting. The position includes supervising Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses to ensure comprehensive patient care.

Responsibilities:
• Conduct comprehensive patient assessments and evaluate medical conditions.
• Develop, implement, and regularly update individualized care plans.
• Supervise and evaluate Home Health Aides and LPN/LVNs, ensuring compliance with care standards.
• Maintain accurate patient records, documenting communication and care activities.
• Communicate promptly with physicians regarding significant patient condition changes.
• Adhere to state and federal regulations governing home health nursing.
• Attend periodic office meetings and coordinate care services.

Qualifications:
• Current, unrestricted RN license valid in the assigned state.
• Minimum 23 years of nursing experience, preferably in home health or related fields.
• Current CPR certification.
• Proficient with computers, including electronic health record systems and basic software navigation.
• Reliable transportation, valid drivers license, and agency-required liability insurance.
• Familiarity with state-specific nursing regulations and standards.
• BSN preferred where applicable.
