Sun River Health is currently seeking a Part-time Medication-Assisted Treatment (MAT) Registered Nurse/Care Manager to join our team in our Church Avenue Brooklyn team. Job Type: Part Time Pay Range: $45.00 - $52.00 per hour $2,700 Sign-On Bonus for Eligible External Applicants – Terms and Conditions Apply Summary: The MAT Registered Nurse/Care Manager is a part of multidisciplinary team within Sun River Health that uses the nursing process to assess, plan, implement, and evaluate patient care and the use of resources. This position will monitor quality of care to ensure that infection control, risk management and all other aspects of services are appropriate. Serve as patient advocate, provide care management, and crisis management. Understand harm reduction approaches and demonstrate empathic understanding of medical and psychosocial development needs. Participate in opioid overdose prevention education and training of staff and patients. Responsibilities: • Act as point person and primary contact champion for the site location • Complete MAT intakes by phone or in person • Participates in all case conference review meetings, department meetings, QI activities, and other relevant agency or community meetings • Complete Recovery Support Plans • Documents all encounters maintaining updated progress notes and case records as required in EMR and relevant data programs. • Complete all necessary consent forms to coordinate care with external agencies • Complete discharge paperwork in conjunction with the MAT provider; Drafting, scanning etc. • Triage presenting issues & provide basic nursing skills in every visit to patients: Complete Pre – visit planning on all charts; load correct templates, update consent forms, address and follow up on open referrals (i.e. SATP, MH/BPS), check ISTOP & PMP (completed in tandem with provider), provide opioid overdose prevention education and training, complete prior authorizations, review and flag labs, collect urine for urine drug screens and pregnancy testing. • Provide care management support to MAT patients: complete and review care management agreement and offer care management support options and linkage (ie health homes, grant funded care management, or nurse care management), connect patients to community resources, follow up on referrals, assist patient in addressing stigma and barriers to care, act as a liaison with pharmacies, conduct patient outreach, monitor progress, and assist with appointment scheduling. Maintain a patient caseload. • Provide Harm reduction education and support to patients and staff including the facilitation of MAT, Fentanyl test strip, and Naloxone training. Complete required documentation for reporting of Naloxone and Fentanyl test strips. Manage and maintain Naloxone and other harm reduction supplies as available for site in coordination with the site Nurse Manager/Health Center Director. Requirements: • BSN - Bachelors of Science in Nursing required • Minimum Work-Related Experience: 0-1 year • Preferred Work-Related Experience: 2-4 years • Valid New York State Registered Nurse (RN) License • CPR/BLS-HCP Certification About Sun River Health Sun River Health has a rich history of providing quality, affordable health care to communities in need.
