Job Description
A company is looking for a Nurse Navigator for the FIND Program, offering a remote opportunity.
Key Responsibilities
Contact patients with incidental findings to offer follow-up options and personalized navigation interventions
Assist primary care providers in ensuring appropriate follow-up for patients based on radiologist recommendations
Maintain communication with colleagues and departments to enhance patient care and program development
Required Qualifications
Graduate of an accredited school of Nursing with a Bachelor of Science Degree in Nursing
Three to five years of progressive clinical nursing experience
Current Registered Nurse license in the State of Michigan
Demonstrated ability to navigate various software applications
Knowledge of Radiology procedures and protocols is preferred
