Nurse Care Consultant/Clinical Care:

Are you a newly graduated RN eager to make a difference? Our Care Consultant position welcomes New Graduates to apply! In this rewarding role, you will conduct assessments, create personalized care plans, and manage the overall care experience for our clients. Mentorship of fellow nurses and collaboration with caregivers are integral to this position. This Full-Time, salaried role features a blend of clinical and administrative responsibilities, with a typical work schedule from Monday to Friday. You will work both in our office alongside the administrative team and in the field conducting patient assessments and case management across the island.

Responsibilities:
• Conduct comprehensive patient assessments and develop individualized care plans.
• Collaborate with healthcare teams to enhance patient care quality and safety.
• Provide expert guidance on healthcare policies and clinical procedures.
• Design and implement staff training and education programs.
• Monitor patient outcomes and recommend improvements.
• Assist in establishing healthcare policies and guidelines.
• Act as a liaison between administrative and clinical teams.
• Foster communication among multidisciplinary teams.

Requirements for Employment:
• State of Hawaii Registered Nurse license, current and in good standing.
• Current CPR & First Aid Certifications.
• Current TB clearance with proof of past TB 2-step test.
• Able to navigate within various home environments.
• Criminal Abstract will be reviewed.
• Pass written entry exams and skills assessments.
• Valid driver's license.
