A company is looking for a Care Manager, LPN (Active New York License Required).
Key Responsibilities
Deliver telephonic care management for patients enrolled in Value-Based Care Programs
Review electronic health records to identify gaps in care and develop patient-centered care plans
Communicate health updates and treatment plans to patients, caregivers, and providers
Required Qualifications
Active, unrestricted compact LPN license
2+ years of nursing experience in various care settings
Knowledge of pathophysiology and treatment protocols for common health diagnoses
Ability to analyze patient records and work in a remote environment
Proficient computer skills and adaptability to technology platforms
