NJ CEED Clinical Case Manager in Monmouth and Ocean County

Position Summary:

The NJCEED Clinical Case Manager is responsible for providing clinical case management and care coordination services to individuals eligible for the New Jersey Cancer Education and Early Detection (NJCEED) Program. This role ensures timely access to breast, cervical, colorectal, and prostate cancer screening and diagnostic services, with a strong emphasis on care coordination, documentation accuracy, quality assurance and compliance with New Jersey Department of Health (NJDOH) program requirements. The Clinical Case Manager works collaboratively with internal teams, healthcare providers, laboratories, and community partners to ensure continuity of care, appropriate follow-up, and resolution of barriers to care for underserved, uninsured and underinsured populations.

Principal Responsibilities:
• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Provide clinical case management and supportive care coordination services to NJCEED-eligible participants in accordance with NJDOH guidelines
• Assess participant needs, identify barriers to care, and coordinate appropriate clinical and supportive services
• Facilitate timely scheduling of cancer screenings, diagnostic, and follow up services, including coordination for abnormal screening results
• Maintain accurate, complete and timely documentation in the NJCEED Cancer Screening Tracking System and other required systems
• Review and process clinical documentation from providers and subcontractors for billing, compliance, and quality assurance purposes
• Collaborate closely with the NJCEED Program Manager, patient navigator, providers, laboratories and community partners
• Monitor case quality and participate in internal and external case audits; respond to audit findings and corrective action requests in a timely manner
• Ensure compliance with NJCEED policies and procedures, performance standards, and reporting requirements
• Participate in quality improvement initiatives to enhance service delivery and participant outcomes
• Provide referral oversight and ensure warm handoffs to internal programs and external community resources as appropriate
• Participate in team meetings, trainings, and ongoing professional development related to cancer screening and public health best practices
• Perform other related duties as assigned in support of program goals

Job Competencies & Minimum Qualifications:
• LPN or RN (BS or BSN) with active license in NJ
• 4+ years of community health, public health, care coordination or clinical case management experience required
• Knowledge of community resources, eligibility criteria and financial assistance guidelines
• Strong computer proficiency, including experience with electronic medical records and data tracking systems
• Excellent verbal and written communication skills
• Strong organizational, time management and interpersonal skills
• Valid Driver’s License
• Bilingual (preferred but not required)

Working Conditions/Physical Demand: Working indoors with client/customer contact with use of phone, computers and medical equipment. Position requires home visitation of clients, extensive paperwork documentation and coordination of services among many disciplines.

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $40,000.00 to USD $80,000.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
