In this role, you will be crucial in coordinating patient care, managing complex health issues comprehensively, and ensuring effective communication among healthcare providers, patients, and their families. By collaborating with a multidisciplinary team, you will help design personalized care plans that integrate both conventional and holistic approaches, ultimately enhancing our patients' health outcomes.We are looking to fill the following shift: Monday to Friday from 8:00am to 5:00pm. Candidates should be willing to commute to our Denver-based office. .ResponsibilitiesConduct holistic assessments of patients to create, implement, and manage individualized care plansCommunicate effectively with patients, providers, and families regarding health management strategies and resourcesProvide education and support to empower patients in managing their health and understanding their conditionsCollaborate with interdisciplinary team members to evaluate progress and adjust care plans as necessaryDocument all interactions and care plans accurately within our electronic medical record systemAdvocate for patients to ensure they receive the necessary services and resourcesMaintain a compassionate and patient-centered approach in all interactions
Hold an active and unrestricted nursing license in the state of ColoradoBring at least 5 years of nursing experience in a busy primary or family care, emergency room, ICU, or rural settingDemonstrate strong clinical assessment and critical thinking skillsDisplay excellent communication and interpersonal skillsBe comfortable with adapting to emerging technology to streamline and improve Amaze's systems.Ability to work independently and collaboratively within a healthcare teamCare coordination or case management in a clinical setting is preferredExperience in chronic disease management is preferredBilingual in Spanish/English - preferredPay Rate is $80,000 to $95,000 - depending on experience
