Details about the Case Manager position we are hiring for: Qualified applicants will need to be approved by NYS DOH Help residents achieve and maintain optimum social, mental, spiritual and emotional well-being. Serve as a resource for residents and their families. Oversee all record keeping practices and ensure they are effective and efficient. Qualifications Collegiate degree and related experience, specifically: A Bachelor’s or Masters’ degree from an accredited college or university in Human Services, Case Management, Social Work or the equivalent, and one year of full-time experience working with the adult or geriatric population; OR An Associate Degree from an accredited college or university in Human Services, Case Management, Social Work, or the equivalent, and three years of full-time experience working with the adult or geriatric population. LPN or RN license Ability to work flexible hours and weekends. Strong organizational and time management skills. Ability to respond calmly to emergencies. Possesses good judgment, problem-solving and decision-making skills. Successfully complete training as required by Meridian in accordance with State regulations. Ability to communicate effectively with residents, families, staff, vendors and general public. Must have compassion for and desire to work with the elderly. Must demonstrate the ability to work responsibly as a team member as well as an individual. Must be honest, fair, dependable, respect confidentiality and the rights and privacy of others and local, state and federal laws. Must practice and promote Meridian Policies and Procedures, Mission Statement, Core Values and Founding Principles. Must meet all health requirements. Must pass criminal background check.
