Job Description

About the Position: This position is located inside Desmond T Doss Health Clinic on Schofield Barracks, Hawaii. This is a Direct Hire Solicitation Duties • Behavioral Health Care Facilitator performs independent, comprehensive telephone and clinical triaging in support of primary care providers within the behavioral health arena. • Facilitate continuity of care through a multidisciplinary approach in the treatment of depression, post-traumatic stress disorder (PTSD) and anxiety disorders in active-duty service members and adult beneficiaries. • Assist clinic with patient and staff education and provides individual and small group training in order to enhance uptake, increase referrals and promote understanding of the role of the behavioral health care facilitator. • Develop appropriate nursing plans. • Coordinate patient referrals. • Make psychosocial assessment of patients to identify significant cultural practices, the ability to understand and cooperate, and expectations of care and coping mechanisms. Requirements • Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation. • Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption. • This position is a Testing Designated Position subject to pre-employment screening and random drug testing. Selectees will be required to consent to participation in random drug urinalysis testing. • A pre-placement medical examination is required. • Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position. • Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS. • Moderate to heavy lifting may be required. • This is a mission essential position: incumbent is expected to make every attempt to report for duty on time and/or remain on duty during severe weather conditions. • May be required to work weekend call duties and evenings. • This position requires regular contact with children under the age 18 and a favorable determination of suitability to do so. • One year probation/trial period may be required.
