This position will receive a short-staffing temporary pay differential of 4% from July 1st 2025 - June 30th 2026

Colorado Veterans Community Living Center (VCLC) at Fitzsimons is a beautiful 180-bed, skilled nursing facility serving Colorado’s veterans. At Fitzsimons, we provide long-term care, short-term rehab and have a secured memory care neighborhood, and are conveniently located on the medical campus with UC Hospital and the Aurora VA Medical Center. Description Of Job

This Nurse II position exists to provide professional clinical services to facility residents including assessment, and specialized nursing care consistent with standards of the profession, legal and regulatory requirements and facility and division standards , protocols and policies. Administers specialized and routine nursing services such as medication and treatments. Documents care provided in the medical record and other appropriate documents. Evaluates residents upon admission and throughout the clinical stay when critical incidents, emergencies, change of condition, or acute illness and behavior changes to identify nursing interventions. Administer IV fluids (if IV certified) as prescribed by a physician. Assess and evaluates the effectiveness of the care/treatment given.

Additionally, As a Nurse II, You Can Expect To
• Plans and coordinates basic nursing care and provides skilled level of nursing to residents. Reviews and audits specific care activities to determine compliance with policies and standards of care as part of a department quality assurance activity. Performs both admission and daily assessments during resident clinical stay, identifying critical stay, interventions, treatment care plan and physician referrals. Document current, accurate and pertinent information regarding care and treatment provided. Detects abnormality in resident’s health status indicated by symptoms, vital signs, assessment and laboratory results and reports information to physician. Provides a broad range of medical and nursing treatments to residents as prescribed within scope of practice and determines most effective methods to ensure optimal outcomes. Utilizes supplies and resources judiciously. Documents treatments and gathers data to determine response to treatment. Participates in work teams to establish best practice protocols for common treatments or procedures. Administers specialized and routine nursing services such as medication and treatments. Administer IV fluids (If IV certified) as prescribed by a physician. Assess and evaluates the effectiveness of the care /treatment given. Explaining patient care before, during and after contacts; answering call lights; and assessing basic resident care needs; updating noted resident changes; recommending routines for patient and family. Provide input and orientation for new employees.
• Competently administers medications to residents within scope of practice while observing the five rights of safe medication administration, resident rights, infection control standards, and appropriate nursing policies. Documents all medications given and justification for scheduled medications not given. Directs and implements nursing care to residents by ensuring compliance with accepted standards of nursing care, departmental and facility policies and procedures and VA and State of Colorado regulatory requirements. Directs subordinates to assure these standards are met
• Provide infection control, implementing/using standard precautions, disposal of waste. Clean equipment, stock supplies, complete supply forms. Identifies high-risk residents for falls, skin breakdown, or other potentially harmful situations and assures appropriate interventions are initiated to correct problems
• Correct use of safety/positioning devices, side rails, transfer belts, proper use of equipment, attend on-going staff education programs. Maintain updated skills, knowledge, abilities and competencies necessary to the position through continuing education, clinical supervision, literature review and participating in discipline and team development
• Provides active listening and communication, support and encouragement to resident’s and families regarding their concerns by both phone and personal contact
• Performs other related duties as assigned

Special Qualifications

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
• Required to obtain a CPR card and possess and maintain a Colorado RN license in good standing

Education/Licensure/Certification/Experience

Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC) Two (2) years of professional experience in an occupational field related to the work assigned to the position

Substitutions

Appropriate education will substitute for up to two (2) years of the required experience on a year-for-year basis

Conditions Of Employment
• CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)
• This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.
• Motor Vehicle Check , if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc.
• Must possess a valid, non-restricted Colorado Driver’s License or an non-restricted U.S. Driver's License for positions within 30 miles of the CO state border
• Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.
• Current, Valid US Driver's License
• Travel: May have to transport residents in state vehicles
• Vaccinations as required by state and facility
• Day, evening, and night shifts to include holidays & weekends as needed
• Former State employees who were disciplinarily terminated or resigned in lieu of termination must:
• Disclose that information on the application.
• Explain why the prior termination or resignation should not disqualify you from the current position.
• Provide your employee number from your prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.

CDHS Selection Process Explanation

Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications. Be sure your application specifically addresses your qualifications, experience, work products, and accomplishments as they relate to the position and minimum requirements.
