Role Description This is a full-time, on-site position for a Nurse Navigator based in Merriam, KS. The Nurse Navigator will guide and support patients through the healthcare journey by coordinating care, providing patient education, and offering emotional support. Responsibilities include assisting with case management, ensuring patients have access to necessary resources, and collaborating with healthcare teams to create a seamless experience. The role involves closely monitoring patient progress and addressing any barriers to care while serving as a key advocate for the patient and their family. Qualifications • Proficiency in Oncology Nursing and advanced knowledge in Medicine • Background in Nursing with expertise in Patient Care • Experience with Case Management processes and tools • Strong communication and interpersonal skills to engage with patients, families, and healthcare teams • Ability to work collaboratively in a multi-disciplinary environment • Current nursing license in the state of Kansas • Oncology Nursing Certification (preferred) • Minimum of 3 years of experience in nursing, preferably related to oncology or patient navigation
