
Nurse Navigator in Phoenix, AZ
Job Description
Looking for someone comfortable with night shifts!
Day to Day:
The Nurse Navigator will work within the Medical Command Center (MCC), a clinical call center that supports 911 systems by triaging low-acuity calls and coordinating appropriate care. Using proprietary clinical decision software (Corti), Nurse Navigators assess patient needs remotely and guide them to the most suitable healthcare resources, helping reduce unnecessary emergency department visits and ambulance dispatches.
Key Responsibilities
• Conduct telephonic triage using evidence-based protocols and Corti software.
• Assess patient conditions and match them with appropriate care options (e.g., urgent care, virtual visits, ride-share transport).
• Coordinate care across a national network of facilities and providers.
• Handle calls from 911, AMR, and Fire departments.
• Route mental health-related calls to crisis lines when applicable.
• Support remote telemonitoring for patients nationwide.
Program Impact
• Improves emergency response times by diverting non-emergency cases.
• Ensures patients receive timely, cost-effective care in the right setting.
• Keeps EMS units available for high-acuity emergencies.
Must Haves:
• Active RN license (unrestricted / compact) -
• Graduate of an accredited nursing program.
• Comfortable managing 25+ calls per shift, including documentation (~11 minutes per call).
• Experience in telephonic assessment and remote patient monitoring.
• Skilled in working with patients of all ages (pediatric to geriatric).
• Strong knowledge of clinical protocols, national geography, and care navigation.
• Ability to multitask and adapt in a fast-paced, dynamic environment.
• Proficient in Microsoft Word and Excel; must be computer literate.
• Minimum typing speed of 30 WPM.
• Excellent phone presence and communication skills.
• Comfortable working with multiple monitors/screens.
