Nurse Navigator- Part Time, Days 8am-4pm (varies 3 days per week, no weekends) Breast Oncology, Overlook Medical Center in Summit, NJ

Job Description Coordinates care for Breast Cancer patients working with the physicians and multidisciplinary team. Principal Accountabilities • Serves as patient and family advocate. • Assists patients with scheduling of appointments to expedite the plan of care and to promote continuity of care. • Assists in education of the patient/family to understand Breast disease process, care map and treatments. • Assists in answering patient calls, initiate call backs and follow-up to meet the needs of the patient. • Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment. Assists with Breast MDCs. • Identifies Barriers to Care and refers to support staff. Participates in Breast Cancer Support Group. • Provides Treatment Summaries' and Survivorship Plans with the physician. • Participates in Breast Programs and community activities. • Performs other related duties as assigned. Required QUALIFICATIONS • Bachelor's in Nursing or related field. • Valid State or national Nursing License. • 2 years of experience in Acute Care. Preferred • OCN preferred. • Oncology experience preferred.
