Responsibilities: About the Role In each visit, which may last up to an hour, Matrix providers assess a member’s current health, including medical history, medication adherence, and social environment, among other risks. This unique evaluation offers insights into a member’s holistic health that can be challenging to obtain in standard office visits. The Matrix Comprehensive Health Assessment (CHA) endeavors to enhance care quality and close several care gaps in one session. Our providers: • Perform adult/geriatric evaluations, covering medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-ups, and documentation following Matrix guidelines and protocols • Administer and order point-of-care and lab tests as necessary • Collaborate with physicians, case managers, social workers, family members, key caregivers, and additional medical personnel when suitable • Work alongside Primary Care Physicians (PCP) on patient education and follow-ups as needed • Conduct services at the member’s residence, whether it’s their home or a Skilled Nursing Facility Benefits of Working at Matrix • Flexibility – Visits are arranged around YOUR available times. Assessments can happen any day of the week between 8 am – 7 pm. Choose to work during the day, evening, or weekends based on your preference. • Competitive Pay – Your earnings depend on the number of assessments you complete. Increase your income by offering more availability or traveling beyond your home location. Those conducting a monthly average of 20–100 visits can earn between $1,800 and $11,500, based on an assessment completion rate of $90–$115, varying by state. • Benefits – Includes mileage reimbursement, 401(k), employee referral bonuses, and optional benefits. • Education and Support – We invest in YOU with comprehensive onboarding training, preceptor assignments, 1:1 feedback, and access to continuing education. • Additional Licensure – If business needs arise, we may assist you in obtaining further state licensure and credentials either in neighboring states or ones you aim to visit for health assessments. • Full-time (FT) Opportunities – PRN providers can transition to FT roles in their area or elsewhere, qualifying for a FT sign-on bonus.
