This role is ideal for an NP who thrives on autonomy, collaboration, and meaningful patient impact providing care where its needed most.Youll visit patients in Assisted Living, Memory Care, Group Homes, and Hospice settings (no personal home visits), managing a census of roughly 90 patients.
