Nurse Practitioner in Owenton, KY

Location: Owenton, KY Schedule: Pay-Per-Encounter | Fee-for-Service | Hybrid Employment Type: W-2 Job Summary Telos Health Systems is seeking a Nurse Practitioner (APRN) to provide mental health evaluations and psychotherapy services to residents in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs) throughout the Clearwater, St. Petersburg, and Brandon areas. This position offers flexibility, clinical autonomy, and strong administrative and leadership support. Key Responsibilities • Conduct comprehensive mental health assessments, physical examinations, differential diagnoses, and manage acute and chronic conditions in accordance with established protocols. • Prescribe medications and treatments as appropriate and coordinate referrals to physicians, specialists, and other healthcare providers. • Collaborate with attending physicians, facility staff, residents, and families to ensure continuity of care, including participation in Advanced Care Planning. • Complete accurate and timely clinical documentation, including appropriate CPT coding, and assist with the development and implementation of patient care policies and procedures. • Perform monthly stability visits for long-term care residents and evaluate patients experiencing changes in condition. Qualifications Required: • Active, clear, and unencumbered Advanced Practice Registered Nurse (APRN) license in the applicable state. • Current DEA registration. • Experience using EMR/EHR systems and proficiency with Microsoft Office applications. Preferred: • Experience working with geriatric populations. • Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.
