What You’ll DoIn this role, you will:Own: Providing in-office and virtual family practice visits, including evaluation, diagnosis, treatment, and follow-up careSupport: Patients by managing acute and chronic conditions and educating them on preventive care and wellnessCollaborate with: Medical Assistants, a Medical Director, and an interdisciplinary care team to ensure strong patient outcomesImprove: Access to primary care through a mix of in-person and telehealth servicesAdvocate for: Patients by delivering thoughtful, patient-first care that removes barriers to treatment

What We’re Looking ForWe’re looking for someone who:Active Arizona Nurse Practitioner (NP) license, required Must be located in Arizona with flexibility to work both in-office and virtually Family Practice or Primary Care experience, preferred Prior telehealth experience, a plus Strong clinical judgment, communication skills, and a patient-first mindset Tech-savvy and comfortable navigating EHR and telehealth platforms Bilingual (English/Spanish) preferred.
