We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, LTC, and ALFs. • Offer comprehensive assessments, manage acute and chronic conditions, and develop treatment plans. • Perform patient exams, order diagnostics, and prescribe medications within guidelines. • Collaborate with care teams to ensure seamless, patient-centered care. • Maintain accurate documentation and support quality initiatives. Qualifications: Current MN NP license, board certification, with 1+ year experience preferred. Strong geriatric and primary care background required.
